The Forest @ Columbia has received a grant to produce a weekly Children’s television show. We will be using footage obtained at the technology center for the program. We will also be starting a Video Production Club (VPC) aimed at our local community teens. Club members will be able to produce their own programs and segments for the children’s program. Please read the information below very carefully. All participants at the center starting October 25, 2007 may be filmed at any time while visiting or using the technology center. For this reason, everyone must now complete a talent release form, which will be kept on file at the technology center. Please complete the fields listed below and provide permission for yourself and any children and teens under the age of 18 years old under your guardianship.

TALENT RELEASE

I understand that audio or videotape may be taken of me and/or my child at any time while visiting and/or using the Forest @ Columbia Technology Center. I hereby assign and authorize the producer MsRevenda.com, People TV Atlanta – Cable Channel 24, & The Greenforest Community Development Corporation all rights in and to such audio or videotape. I also authorize said producer, without limitation, the right to reproduce, copy, exhibit/publish, and distribute any such audio or videotape, and waive all rights or claims I may have against your organization and/or any of its Affiliates, Subsidiaries, or Assignees other than as stated in this agreement.
